These go all the way back to Pearl Harbor and before. They always involve the same problems of failure to share and failure to understand the information in hand. None of the reforms of the last 70 years have made any impact on the agencies. This leaves a large gray area for all kinds of ugly operations and policies to take place - no one wants to know.  It amounts to a fundamental courrption of intent, this in turn, creates an environment of distrust over and above the natural distrust intrinsic to an intelligence agency. We really can't claim to be surprised by this behavior, it's happened over and over again since Bill Donovan created the OSS. We've never taken effective oversight action so why would we expect our intelligence community to act 'responsibly' when there are apparently no repercussions for the endless CYA dance?
